Metformin HCl

Type 2 DM as monotherapy. Adjunct to diet & exercise to improve glycemic control.

Monotherapy & in combination w/ other oral hypoglycemic agents/insulin Adult Initially 750 mg. Max: 1,500 mg. Adjust dose based on blood glucose measurement.

Should be taken with food: Take w/ the evening meal. Swallow whole, do not chew/crush.

Hypersensitivity. Intravascular administration of iodinated contrast media; acute or chronic metabolic acidosis including diabetic ketoacidosis w/ or w/o coma; acute hemodynamic compromise of hypoxic states; CHF; dehydration. Renal disease or dysunction resulting from conditions eg, CV collapse (shock), acute MI, septicemia; abnormal hepatic function. Lactation.

Lactic acidosis can occur due to metformin accumulation. CV collapse (shock), acute CHF, MI & other conditions characterized by hypoxemia. Temporarily suspend therapy for surgery except minor procedures not associated w/ food & fluid intake. Excessive alcohol intake known to potentiate effect of metformin on lactate metabolism. Annually measure hematologic parameters. Hypoglycemia may occur w/ caloric intake deficiency, strenous exercise not compensated by caloric supplementation, or concomitant use w/ other glucose lowering agents (eg, sulfonylureas & insulin) or ethanol. Temporary loss of glycemic control w/ exposure to stress eg, fever, trauma, infection or surgery. Hepatic impairment; monitor renal function in elderly >80 yr. Elderly, debilitated or malnourished patients; patients w/ adrenal insufficiency & those taking β-adrenergic blockers. Not recommended for routine management of gestational DM, type 2 DM in pregnant women or polycystic ovary syndrome. Pregnancy.

Diarrhea, abnormal stools, hypoglycemia, myalgia, lightheadedness, dyspnea, nail disorder, rash, increased sweating, taste disorder, chest discomfort, chills, flu syndrome, flushing & palpitation.

Enhanced absorption w/ nifedipine. Cationic drugs eg, amiloride, digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamterene, trimethoprim, or vancomycin may compete for common renal tubular transport systems. Increased peak plasma & whole blood conc w/ cimetidine. Risk of hyperglycemia & loss of glycemic control w/ thiazides & other diuretics, corticosteroids, phenothiazines, thyroid products, estrogens, OCs, phenytoin, nicotinic acid, symphatomimetics, Ca channel blockers, INH.
